Make It Impossible, Not Just Forbidden: The Quiet Genius of Mistake-Proofing

The best safety decision a designer ever makes is usually invisible, because when it works, nothing happens. Consider the humble SIM card. It has one clipped corner, for exactly one reason: so it can only go in the right way round. You have never read a SIM-insertion warning label, because you cannot insert it wrong. That little clipped corner is a small masterpiece of a discipline called poka-yoke — Japanese for "mistake-proofing."

Coined by the engineer Shigeo Shingo as part of the Toyota Production System in the 1960s, poka-yoke — also called a forcing function — is the practice of shaping a process or an object so that the wrong action is either impossible or instantly obvious. Once you see it, it is everywhere: the three-prong plug that will not go in upside down, the microwave that refuses to run with its door open, the car that will not leave Park until your foot is on the brake, the diesel nozzle sized so it physically will not fit a petrol filler, the USB-C connector that works either way up. Whole catalogues of these tricks exist, and the best of them separate prevention — designing out the very possibility of the mistake — from mere detection, which only catches it after the fact.

Here is the design-intelligence core, and it is uncomfortable: a warning label is an admission that the design failed to prevent the error. Don Norman said it plainly in The Design of Everyday Things — when a device as simple as a door needs an instruction manual, it is badly designed. Every "CAUTION" sticker, every "Are you sure?" dialog, every manual line that begins "Do not," is a fossil: the visible trace of a moment when a designer could have made the mistake unavailable and instead chose to warn against it. Warnings are cheap and geometry is expensive, so most organisations warn.

It helps to see the full ladder of responses to a possible error. One: make it impossible — the shape or the interlock simply forbids it. Two: make it hard — a guard, a cover you must lift, a deliberate two-step. Three: warn against it — a label or a dialog, which is weak because it leans entirely on a human reading and heeding it, usually while stressed or distracted. Four: blame the user — "operator error," the pure abdication. The cheapest rungs are the bottom two, which is exactly why they dominate every product you find frustrating. But each rung you climb moves the safety out of the fallible human and back into the reliable object, where it belongs.

The deepest move poka-yoke makes is to quietly reframe the whole relationship between people and things. If users keep doing the "wrong" thing — plugging the cable in backwards, leaving the machine in the wrong state, misreading the one control that matters — the design invited it. Norman's argument about affordances is that a good form tells you what to do and refuses what you should not; the mistake is information about the design, not a verdict on the person. As the error-proofing literature keeps repeating, "be more careful" is not a countermeasure — it is what you say when you have run out of design.

And this is the part that matters most: mistake-proofing is a concept-phase decision, never a finishing one. The clipped corner has to live in the SIM's geometry from the first drawing; the interlock has to be built into the mechanism; the asymmetry has to be in the connector's cross-section. You cannot poka-yoke a finished product with a sticker — you can only warn it. So the question that separates a robust design from a fragile one gets asked right at the start: for each way this thing can go wrong, can we make that wrong unavailable — or are we about to spend its entire life warning against it? Designing the error out is upstream work or it is nothing.

None of this means you design out every possibility. Over-constraining kills the flexibility real use needs, and today's "error" is sometimes tomorrow's valid edge case; the discipline is judgment about which mistakes are catastrophic enough to forbid in the form itself. But the instinct is the tell. A mature design culture reaches first for "make it impossible," and only when that genuinely fails does it reach for the label. The warning sticker is the exact line where design stops and hope begins — and hope is not a safety system.
